Ohio Premier Registration Expo for the 2008-09 season will be held Saturday, June 28th at Dublin Coffman HS commons.   Please plan to attend during your child’s age group.   Times are as follows:

8:30-10:30 am     U8-U10

10:30-11:30 am   U11-U14

11:30-12:30 pm   U15-U18

Upon acceptance to your team, please  begin your online registration by clicking on the Player Account/Registration link on left.  It is imperative that you fill out all information requested, particularly email address as this is our only source of communication.  If you are a returning player, you will need to UPDATE YOUR INFO and select your new team for the 08-09 season.  If you plan to pay your deposit ONLINE, please be sure to print out the payment receipt and bring with you to registration as deposit confirmation.

THE FOLLOWING FORMS WILL NEED TO BE COMPLETED AND BROUGHT TO THE REGISTRATION EXPO.   All forms can be found under FORMS link as well as APPAREL/UNIFORMS link on the left side.  If for some reason you are unable to attend, please send materials including deposit and/or deposit receipt to OP, PO Box 56, Dublin, OH  43017.

• USYSA Membership Form
• Medical Release (NEEDS TO BE NOTARIZED)
• Ohio Premier Liability Release (only if NEW to Ohio Premier)
• Uniform Order Form (Either Parent Coached or Professionally Coached as necessary)

You will also need to bring the following with you along with the above forms:

• One small thumbsized picture of player
• Copy of birth certificate if new to Ohio Premier this year (otherwise we will use 07-08 player card)
• $500 deposit (prof coach teams except U8 which is $200) $287.50 (prof assisted teams) unless paid online in which case you should bring computer generated receipt.
