GOALS OF OP EAGLES FALL TEAM:

1) To provide players an enjoyable and highly challenging experience where they are pushed to play at the highest level possible. 
2) To gain significant exposure with college coaches by competing in tournaments where coaches are recruiting.

Ohio Premier Eagles is forming 1 or 2 teams of high school age players for the fall 2011 season.     

Playing with OP Eagles during the fall of 2011 means that you cannot play with your high school team. Playing with your high school team means that you cannot participate in OP Eagles activities.   This is not an Ohio Premier Eagles policy, but rather the rule of the Ohio High School Athletic Association.

Please return this form if interested in playing for the fall team. All forms must be received no later than 6/17/2011 accompanied by a $200 non-refundable deposit (unless not chosen to team or team is not formed). Your form will not be considered without the deposit check. Total fees for fall team participation will be $500 (includes $200 deposit).   There will also be a minimal uniform charge for t-shirts of roughly $30.

 

Please note:

·        Your decision will not affect your future in OP Eagles programs for the winter/spring teams. This team is separate from the age group teams that begin play after the high school season is completed.

·        A decision to play for the fall team is not a guaranteed spot on the team.   There may still be selections and/or tryouts.

·        If you check that you wish to play for op eagles fall team, your signature and a signature of a parent/guarding is required.

·        A non-refundable deposit must be included with this response sheet to be considered for the team.   This deposit will be returned only if the player is not selected by the OPE staff to play on this team.   If the player does not play for the fall team for any other reason after selection, the deposit will not be returned.
