Attention OP Eagles Players:
I am an OP Eagles parent and a faculty at The Ohio State University College of Dentistry. I would like to offer OP Eagles players a mouthguard made to fit your athletes’ mouth at no charge. I will be using the same PolyShok material worn by The Ohio State Buckeyes. In order to take advantage of this opportunity I will need a stone model of your players upper jaw made from an alginate impression by your dentist. Please keep in mind that your athletes’ dentist may charge you a fee for the impression. It depends on the individual office. Please tell your dentist that I need the stone cast and not the alginate impression itself. This will make sense to the dentist office if you take this form with you.
What if your child wears braces? If your child wears braces, you will need to have the orthodontist make an impression with the archwire in place. This will also make sense to the orthodontist office if you take this form with you. Remember that your child’s teeth are in the process of moving with the braces so it is probably best to wait to have the impression made until closer to the soccer season.
Once you have obtained a cast. Please follow the instructions below:
There is a drop box located at the front counter in the indoor facility where you can drop off the stone model and pick up the completed mouthguard. Please be sure to place the model in a small box or container so that it doesn’t become damaged. You will need to include the players name and team. I will stop by every week to check for models. The anticipated turn around time is about 10 -14 days.
